1. Any stoppage of play caused by an attacking player in the attacking zone (penalty, puck shot onto netting, puck shot outside rink, goal cage dislodged, puck frozen on boards)
2. Attacking player in the goal crease
3. Puck carried offside
4. Coincident icing (defending team) and delayed penalty (attacking team)
5. During a stoppage of play and a gathering of players, any attacking player crossing the tops of the end-zone face-off circles
6. Offside error by officials
