Student Rating Form

Welcome to the CDIHOA Mentoring Program Student Rating Form. This form will allow you to rate the student in four simple parts:

In Part A , you will enter information about yourself, including your full name and e-mail address. This will allow CDIHOA to keep a record of your submission, and to contact you if a follow-up is necessary.

Note that anonymous submissions will not be accepted, since CDIHOA must be able to contact you regarding your feedback. However, you can request that your name and any other identifying information be withheld if CDIHOA contacts the rated official.

In Part B , you will enter information about the game in which you observed the student, including the date, time, location, and level. The location is selectable from a list of rinks in the region. The game level will also be selected from a list.

In Part C , you will enter information about the student being rated. You will be presented with several categories for which a rating may be provided, such as "Appearance", "Hustle", and "Positioning".

Finally, in Part D , you will verify that all of the information that you entered is correct, and submit your rating form to the appropriate CDIHOA personnel.

Note that in each part, required fields will be displayed in bold face, while optional fields will be displayed in the standard font.
